How To Enroll

To be eligible for a Physicians United Plan Medicare plan you must meet the following requirements:

  • You must live in our service area

  • You must be entitled to Medicare Part A (hospital insurance) and enrolled in Medicare Part B (medical insurance), and continue to pay your Medicare Part B premium (and Part A if applicable), if not paid for by another third party.

  • Individuals with end-stage renal disease (ESRD) are not eligible to enroll unless the individual is currently a member with Physicians United Plan or if their previous Medicare Advantage Plan was terminated. An individual who receives a kidney transplant and no longer requires a regular course of dialysis to maintain life is not medically determined to have ESRD.

  • Physician United Plan’s contract with CMS is renewed annually and availability of coverage beyond the end of the current contract year is not guaranteed. Members may enroll in the plan only during specific times of the year.

You may enroll in the plan only during specific times of the year.

    You may also enroll online through the Centers for Medicare and Medicare Services (CMS) website. Click here to access the CMS Online Enrollment Center. Follow the directions provided on the site to complete your enrollment. CMS will send us your application electronically.

    If you have any questions, please call Member Services at 1-866-571-0693 (TTY/TDD 1-866-671-0693). Our hours of operation are Monday through Sunday 8:00 a.m. to 8:00 p.m.

    *Paper copies of information posted on our website are available upon request.

    You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, call:

    1. 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048, 24 hours a day/7days a week); OR


    2. The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call, 1-800-325-0778; OR


    3. Your State Medicaid Office.
